48-year-old man presents to you complaining of palpitations

A 48-year-old man presents to you complaining of palpitations for the last hour. He is alert and oriented, his respiratory rate is 18 breaths/minute and minimally labored, and his blood pressure is 90/68 mm Hg. Electrocardiography (ECG) is obtained (see Figure).

Which of the following statements describes the most reasonable next step in the management of this patient's condition?

  • Use cardioversion as a last resort because this rhythm tends to unfavorably respond to electrical therapy.
  • Give adenosine as a diagnostic agent.
  • Obtain a cardiac consult.
  • Attempt pharmacologic rate control with diltiazem or a beta blocker and cardiovert if this is not successful.
  • Start amiodarone, which is now preferred over magnesium for treating this rhythm.
