Most Missed Question in Family Medicine Prep – Pituitary Hyperplasia

Severe primary hypothyroidism can produce a sellar mass–appearing pituitary enlargement via TRH-driven pituitary hyperplasia, and the labs (very high TSH + low free T4) make the diagnosis.

Question – Sellar mass

A 32-year-old man with no previous medical history complains of frontal headache for 3 months, unintentional weight gain of 10 pounds, and cold intolerance. On examination, his blood pressure is 120/60 mm Hg and his pulse is 60 beats/minute. His thyroid is not enlarged. His thyroid-stimulating hormone (TSH) level is 464 mIU/L (normal range, 2.5-4.0 mIU/L), free thyroxine (T4) is 0.1 ng/dL (normal range, 0.7-1.9 ng/dL), and free triiodothyronine (T3) is 75 pg/dL (normal range, 230-619 pg/dL).Magnetic resonance imaging of the brain shows a sellar mass extending into the suprasellar cistern with mild compression of the optic chiasm (see Figure).

What is the likely cause of this patient’s sellar mass?

Answer Options:

A. TSH-producing adenoma  
B. hypothalamic lesions

This keyed answer is correct by current standard endocrine diagnostic principles: the biochemical profile (TSH 464 mIU/L with profoundly low free T4/T3) is unequivocal primary hypothyroidism. In severe, prolonged primary hypothyroidism, loss of negative feedback increases hypothalamic TRH, which can drive pituitary thyrotroph hyperplasia (and often lactotroph hyperplasia), producing apparent pituitary “macroadenoma” imaging findings and mass-effect symptoms (e.g., headache, chiasmal compression).

This is a classic board trap: test-takers anchor on “sellar mass = pituitary adenoma” and ignore the thyroid axis pattern. - “Mass on MRI” triggers reflex selection of pituitary adenoma despite thyroid labs that strongly localize disease to the thyroid gland.  
- Confusion between TSH-secreting adenoma (high TSH *and* high T4/T3) vs primary hypothyroidism (high TSH with low T4/T3).  
- Under-recognition that pituitary enlargement can be reactive hyperplasia and may regress with levothyroxine.
